Transcript Request Form

Complete all sections. Delays will occur during peak periods, such as registration, graduation, and at the end of the semester. May also get a form to return by mail, fax or in person.

The registrar will issue one official copy of the transcript without charges to current and former students who are free of financial and other obligations to the School. Fees are charged for additional copies of transcripts, or expedited same-day service. If this is the first copy you request, please make a formal request through registrar@ost.edu. (Please include delivery preferences and mailing address if necessary)
