Ministry to Minister Sabbatical Program Online Application Forms

Please submit the following four forms: Program Application, Ecclesiastical Sponsor, Confidential Recommendation, and Medical Information Form.

What you need to apply:

Application Form – Files can be uploaded later on the file upload page
- Autobiographical Sketch
- Recent Photo
- Copy of COVID-19 Vaccination Record
- Copy of Passport (Only International Applicants)
- Application Fee $55 (Can be paid with the application form or after on the payment page)
Two Confidential Recommendations
- 1st Recommendation: From Ecclesiastical Superior
- Diocesan Priest – Recommendation From your Bishop or Vicar of Clergy
- Religious – Recommendation from your Provincial Superior or a member of the provincial council
- Lay Ecclesial Minister – Recommendation from your current ecclesial employer or ministerial supervisor
- 2nd Recommendation may be from a friend, colleague, family member, etc.
